Submit school forms through our patient portal in May or early June and allow at least two weeks turnaround time. Forms not submitted by the end of July may be assessed a processing fee due to high volume of requests and provider availability.
To submit, log into the Patient Portal and go to "Ask a Question," select Doctor Care Team, fill in the required fields for the request, and attach the forms.
