My child has turned 18. How do I get permission to talk with you about their care?

Once a patient turns 18, they become their own guarantor, which requires a separate account and a separate statement.

If you want to discuss your child’s treatment with us after they turn 18, they must complete a Medical Information Release Form granting us permission.
